The documents, fees, and procedures will vary for the renewal of passport depending on the individual. For a person who is under the age of 14, the parents should accompany the kid. A recent passport size photo, birth certificate, parental identifications and a filled up DS-11 form should be submitted along with the application fee. If you were under 16 when your passport was issued then you have to appear in person along with the completed DS-11 form and a recent passport size photo and your passport.
Unless you are requested, do not sign in the DS-11 form. The consular officer will request you to sign after verification. If you were more than 16 years of age when the passport was issued then you have to fill up DS-82 form also along with DS-11.
